Here is a list of all the forms offered by the Pocahontas County Assessor’s Office.
Address Change Form
Use this form to change the mailing address that your tax bill is sent to. You must be the legal owner or have the power of attorney of the legal owner to request an address change. The required information such as Account Number, District, Map and Parcel can be found at the top of your tax billing statement. If you do not have your statement available, you can download one by searching the Sheriff’s Tax Inquiry website. You can return the form by mailing it to the address or faxing it to the number at the top of the form.
Farm Use Application
Stop by the office or contact us to request a farm form. If you farm yours or someone else’s land, you must fill out a Farm Use Valuation Application Form each year beginning on July 1st. The form must be returned to the Assessor’s Office no later than September 1st each year. You must have a minimum of $1,000 of agriculture production (or $500 if you are farming less than 5 acres of land) to receive the tax benefits of farm class land.
Stop by our office or contact us to request an application for homestead exemption. If you are 65 or older or are permanently and totally disabled, you may qualify for a homestead exemption of $20,000 off the assessed valuation of your residential property. Return this application for homestead exemption before December 1st. You only need to apply for homestead exemption once, so if you have applied for and been approved in the past, you do not have to file again unless you move to a new residence.
Personal Property and Real Estate (including Dog Tax)
A Personal Property and Real Estate Report is due by October 1st of each year. We begin Assessing Personal and Real Property beginning on July 1st of each year. We attempt to make a personal visit to each resident in the county, but if we have missed you, you can download this form and return it to our office. Dogs, Sheep and Goats are also assessed with your personal property.

Property Improvements Form
If you have made any improvements that have increased the value of your property by more than $1,000 you must notify the Assessor’s Office within sixty days from the date the work is completed by submitting the Notice of Building or Real Property Improvement form.
