This form is to change the mailing address that your tax statements are sent to. To submit a change of address request you must be the property owner or have legal power of attorney of the property owner. You will need to have the account information for the property you are requesting this address change on. You can find this information on your tax statement or receipt. If you do not have one of these available you can download a copy by searching the Sheriff’s Tax Inquiry website here. By completing this form, you are certifying that you have the authority to submit this request.
